Prompt and Kno2 bring connected care to rehab therapy and chiropractic clinics

Prompt Health's new partnership and integration with Kno2 gives rehab therapy and chiropractic clinics the interoperability foundation they need to participate in connected care, reduce administrative burden, and improve the referral experience. Kno2 leads the future of healthcare communication and is a federally designated Qualified Health Information Network (QHIN) and CMS Aligned Network.

The referral and communication gap in outpatient rehab

Outpatient rehab and chiropractic clinics play a critical role in the healthcare system, yet many remain disconnected from the electronic networks hospitals, health systems, and physician groups use to exchange referrals and clinical information. As healthcare shifts toward secure, standards-based interoperability, many clinics still rely on fax and manual workflows to receive referrals and exchange patient information.

Referral information often arrives by fax, gets saved as a file, and then parts of it are re-entered by hand into the patient chart. These disconnected workflows create unnecessary work for staff, slow referral processing, make it harder to keep referring providers informed, and can leave patients feeling removed from their broader care team.

The result is friction for everyone involved in the referral process:

  • Admin teams deal with duplicate documentation, chasing faxes, and questioning what was sent and what wasn't.
  • Clinic owners face administrative overhead and referral risk that compounds across every provider and location.
  • Patients experience care that can feel disconnected from the team that sent them, with no reliable way to know their full care team is on the same page.

Modern interoperability, built into Prompt

Built directly into Prompt, the integration with Kno2, a full-stack communication network purpose-built for providers, connecting clinical systems, data, workflows, and communication pathway, helps clinics reduce administrative burden, lower referral management costs, and deliver a more connected experience for patients and providers alike.

Instead of relying on manual referral workflows, clinics can:

  • Automatically receive inbound referrals as structured C-CDA documents instead of by fax.
  • Automatically route referrals to the appropriate clinic location based on patient geography.
  • Securely send and receive Direct Secure Messages (DSM) with referring providers, hospitals, and other healthcare organizations.
  • Send and receive C-CDA documents (structured clinical records), allowing providers to exchange patient records electronically across organizations.
  • Review and attach structured clinical records directly to the patient record, eliminating manual scanning and re-entry.

By connecting Prompt clinics to the same interoperability networks hospitals and physician groups already depend on, teams can communicate more reliably with referral partners, reduce manual follow-up, and participate more fully in connected care. Through Carequality, CommonWell, TEFCA, and Direct Secure Messaging (DSM), Prompt clinics can securely exchange referrals and clinical information with more than 6 million providers across over 80% of certified EHRs. Unlike fax, delivery is confirmed, failures are flagged immediately, and referrals are less likely to fall through the cracks.

Many interoperability solutions in healthcare were designed around large hospital systems or generalized EHR workflows. Prompt’s partnership with Kno2 is built specifically for the realities of outpatient rehab therapy and chiropractic care. Instead of treating interoperability as a disconnected technical layer, Prompt embeds communication workflows directly into the operational and clinical experience teams already use every day.

For enterprise rehab and chiropractic organizations managing high referral volumes across multiple locations, these capabilities are increasingly important for reducing administrative overhead and costs, protecting referral relationships, and competing effectively in coordinated care environments.

"Rehab therapy has always been an essential part of how people recover and get back to living their lives. Prompt was built to modernize how those clinics operate and Kno2 is how we connect that work to the rest of the healthcare system. For clinic owners, that means less time fighting administrative friction, better communication with the referral partners sending them patients, and a foundation that positions their practice for where healthcare is heading," Adam Baliatico, Co-Founder, Prompt Health.

Built for the future of connected care

Best of all, the Kno2 integration is designed to deepen over time. As it does, Prompt clinics will be positioned for deeper hospital and health system connectivity, bidirectional referral workflows, and participation in federal interoperability programs.

Prompt has also signed the CMS Health Tech Ecosystem pledge, aligning with the CMS Interoperability Framework's standards for secure, patient-centered data exchange.

Future capabilities will further automate referral workflows by matching incoming referral documents to patient records and extracting key referral information into structured chart fields for staff to review and approve before updating the patient record. Referring providers will also receive status updates as patients move through care, from first contact through discharge, creating a closed-loop referral experience that strengthens referral relationships over time.

And as Prompt expands native querying capabilities, clinics will be able to query Carequality, CommonWell, and TEFCA directly within Prompt, building on the connectivity already available through Kno2 today.

Turning outcomes into opportunity

As Prompt's outcomes platform matures, the Kno2 integration becomes the channel through which structured outcomes data, including functional progress, pain scores, and discharge status, flow back to referring physicians, hospitals, ACOs, and payers. That closed loop is what positions Prompt practices as full participants in outcomes-based care, not just downstream recipients of hospital referrals.

For practices building toward ACO partnerships, MSK bundled payment programs, and preferred-provider arrangements, that outcomes layer is what makes the difference. The practices that build this foundation now will be the ones best positioned as those expectations become the standard across the care continuum.
