“Oversight on everything”: How Prompt helped Edge Physical Therapy & Sports Medicine boost revenue per visit by 25%

Dr. Scott Bremer, DC has been practicing for over 25 years, and he’s owned his own business for around 20. His current practice, Edge Physical Therapy & Sports Medicine, is a multi-disciplinary clinic with 3 (soon to be 4) locations throughout New Jersey. 

Between these locations, he has up to 10 physical therapists on staff, with an additional 4 full-time chiropractors. And between these providers, Edge can see anywhere from 70-100 patients a day. 

With such high volume clinics, Scott needs clear insight into every aspect of the business.

The breaking point: denials, duplication, and lost time

Scott had long used a chiropractic-specific EMR that he thought was great 5 or 6 years ago. But, in recent years, insurance carriers started denying more of his claims and he wasn’t getting the support he needed to address those issues. 

“I started getting more denials, and I lost probably $3,000 in the first month of that happening,” Scott said. 

To address the issues, Scott had to do a lot of manual review, on top of his full caseload of patients. “I used to have 3-hour sessions after the workday on Thursday to review the week of what was done, what wasn’t done. It is exhausting to deal with all that after a 10-, 12-hour day,” he said. 

Making the switch to Prompt

The lack of support and timely response from his previous EMR was the final straw. “If  the software company I literally am paying, they make the mistake and they won't address it, that’s not acceptable to me,” he said. 

So he began looking for a new EMR. As one of the first chiropractors to use Prompt, he found it challenging in the beginning. “My business partner is a physical therapist and he loved Prompt, but there were a lot of things that didn’t apply to what I do,” Scott said.

But the support and product evolution Scott has experienced as Prompt adapted to chiropractic nuances convinced him to stick around. And he's happy he did.

Early on, the team quickly addressed a core gap (spinal manipulation missing from the modalities list). “I probably brought it up Tuesday, and by Thursday afternoon it was fixed. I’ve never in my life even expected a response that fast,” Scott said. 

As an early chiropractic adopter, Scott has continued to give feedback to the Prompt team, and “they have listened to every single issue I’ve had and they addressed it. I’ve never worked with any company like this.”

Two years later, Edge is still using Prompt, and seeing results.

What changed: accountability, clarity, and clean billing

The biggest impact showed up where Scott needed the most leverage: multi-location oversight and multi-discipline billing.

With all providers of multiple specialities documenting in one system, Scott is able to see what was billed, what wasn’t billed, who’s not completing their notes, and track units billed per visit in real time. 

The first time he saw the billing screen in Prompt, Scott says his “jaw dropped that it was even possible for what I was asking for. It’s one thing for one provider, but I have 20 providers across multiple different professions. To have it all in front of me at a touch is really nice. It’s made my life a lot easier.” 

And when something falls through the cracks, he can immediately spot it and take action. “The proof is right in front of you. It takes under 10 seconds to find and address it,” he said. 

Critically, Prompt’s billing automation flags any potential problems that would cause denials, like when similar treatments are being given to one patient on the same day. 

Having these alerts, Scott said, allows them to adjust and add to or change their treatment plan so they can maximize the codes they are able to bill.

And as a result, “Per visit, we’re billing 25% more than we were. The money has gone up across the board for the entire practice,” he said. 

Keeping schedules full: automated outreach that adds real visits

For his clinics serving acute injuries and post-op patients, open slots are an opportunity for Edge, if the right patients know about them. Prompt’s automated waitlist outreach fills those gaps.

“Prompt sending out automated reminders when there’s an opening in the schedule to patients who need it has made a big impact, both on the number of patients we’re seeing and in the amount of money we’re generating,” Scott said. 

As a result of these waitlist automations, Edge can schedule up to an additional 10 patients in a day. “It adds up, and it’s significant.” 

Getting his nights and weekends back

Operational visibility and automation didn’t just improve the numbers; it changed the rhythm of Scott’s week.

“It’s shortened my nights, and it’s given me at least the majority of my weekends back because I don’t have to go through things. I don’t have to double check what’s been done or what hasn’t. I’ve slept better, because I’m not constantly in my head wondering if I’m missing something,” he said.  

From a quality of life perspective, Scott said, “I have trouble putting it into words what a big difference it’s made for me.” 

Continuous improvement (that shows up on the bottom line)

Scott ties the continued success of his practice since switching to Prompt’s feature release cadence and openness to feedback.

“They do almost weekly software updates. There’s so many things that they’ve been introducing, and every one of them across the board makes me more money and makes my life easier,” he said. 

He’s clear about why that matters as he continues to expand: an Edgewater location is next, with Hoboken on the horizon. “I don’t know if I would ever even attempt another company of this size with this many locations with a different software,” he said. 

The takeaway

What started as a challenging switch has turned into a growth engine for a complex, high-volume, multi-disciplinary practice. 

The combination of accountability, crossover prevention, and automated schedule filling has translated into at least 25% more revenue per multi-provider visit, up to 10 additional visits per day in rehab-heavy locations, and the return of Scott’s time that used to disappear into after-hours audits.

Or, as Scott puts it: “It’s probably the greatest software initiative ever taken that I’ve ever seen in my 26-year career.”
