Manage user permissions in bulk

Access control gets harder to track as a practice grows. What someone can see and do in Prompt comes down to which permission group they belong to. New clinicians start, people change roles, and those groups need updating.

Permission groups now filter those groups by provider, provider type, and membership. Add or remove people in bulk instead of one edit at a time.
