Why Tulsa Spine & Rehab switched EHRs after 13 years
For years, Sean's team fought a system that couldn't keep pace with the practice's volume, and support meant getting passed from person to person with no real answers.
"13 years with my previous EMR. It was challenging. I have quite a few issues with them. It was Band-Aid after Band-Aid and duct tape and constantly fighting. We really struggled with support," Sean said.
The old system was also server-based. Sean couldn't document from home or on the road, and his providers couldn't check schedules remotely. Storms knock out power and network access often in Tulsa. Each time that happened, the whole practice lost the ability to reschedule patients. He paid his IT provider for ongoing backups on top of it, a cost that only grew as his patient volume did.
"[My old EHR] never changed for me. It was static. It was, “this is what we have.” And over the course of 12 to 13 years, it really didn't change much for us," Sean said.
That static experience showed up in the practice's toolkit, too. Sean paid separately for roughly half a dozen a la carte services his EHR didn't cover: Dragon dictation software at $300 a month, third-party texting at $500 to $600 a month that changed ownership repeatedly and never synced with his EHR, and an intra-office messenger with no record of who did what.
"I paid half a dozen a la carte services that I was required to pay," Sean said, and the cost was for a system that, in key ways, still didn't work.
Billing that didn't protect his revenue
The disconnect between documentation and billing cost Sean directly. His old system let him bill 4 units of care to payers that only covered 2, meaning he wrote off half the visit without realizing it until it was too late. This reflects Sean's specific payer mix and coverage rules; unit limits and coverage vary by payer and plan. Verification of benefits also failed roughly 75% of the time, leaving staff to guess what to collect and patients to build balances that fed directly into Sean's aging AR.
Documentation had consequences beyond the day-to-day, too. A missed signature meant a claim never went out, and at audit, Sean would sometimes find unsigned notes from 18 months back.
"The note is the revenue source," Sean said. "If I don't sign the note, I can't get the billing out."
Does Prompt work for chiropractic practices?

Collecting up front instead of chasing it down
One of the bigger shifts at Tulsa Spine & Rehab has been moving collections to the front of the visit instead of the back end. Sean was initially resistant, worried it would feel too transactional for a practice built around a high-touch, service-forward patient experience. His team now uses Prompt's alerts to know exactly what to collect before the patient ever leaves the front desk.
"We're winning there as well. Once again, I attribute that to the software, the communication, the text messaging, the emails," Sean said.
Only a small fraction of patients ever raised an issue with paying up front, and the shift, combined with automated reminders, has also brought Sean's no-show rate below 8%, something his old system's third-party communication add-on could never reliably deliver.
97% of AR in 30 days, up from 80%

"97% of our AR is zero to 31 days. It's unheard of. And I would say that with my previous software, zero to 30 was 80, 82%," Sean said.
Revenue collected per visit is up 22% over the same period. "It's unheard of. You start doing the math on that, that's real revenue," Sean said.
Documenting in 90 seconds with Sidekick
For a provider seeing this many patients a day, Sean credits Sidekick, Prompt’s AI note-taker, with changing how he documents. He now dictates a note in about 90 seconds and lets Sidekick clean it up and format it into his SOAP notes. He also uses Prompt's AI Intake to pull patient intake information directly into the note, another feature he called a big time-saver.
"I can just talk now, and Sidekick does a really nice job of cleaning up the note, but also formatting it in a way that fits that SOAP note," Sean said.
He relies on the rewrite feature daily to keep notes compliant on recurring visits, where little changes from one appointment to the next but notes that read the same can draw audit scrutiny. His previous software gave him no way to vary the wording while keeping the clinical detail accurate.
"That just increases our efficiency to the next level," Sean said.
From guessing to knowing: Sean's 6-month business review
